NOTE: The application form is a PDF fie that is downloaded locally to your computer. THERE IS NO WAY TO SUBMIT THE FORM ONLINE. You can print it immediately and then write in the fieds by hand, OR you can type directly into the PDF form on your computer and then print it. No matter how you complete the form, it will need to be printed out and brought into the store.
